When to use a GOS4 'repair or replacement' voucher
Any element of the GOS(S)4 optical voucher system which normally requires approval from an NHS Board remains suspended until further notice.
When completing any GOS4 claim, please indicate whether you are claiming for a repair or a replacement. Also make sure it's clear which part(s) of the spectacles you are claiming for.
