Request or amend practice eSchedule contact access

If you're a General Dental Practitioner or Orthodontic Contractor you may request eSchedule contact access. By completing this form this will give you access to all dentists within your practice eschedule reports. Please ensure that you have agreement from the current eSchedule contact within the practice prior to requesting this as their access will be removed.

Please complete the Dental online reporting eSchedule contact OLR002 form

Once this form is complete, please return it to: nss.psd-customer-admin@nhs.scot. The form must be sent directly from the General Dental Practitioner or Orthodontic Contractor using their NHS email.
