Prior Approval Reform – Tiers Explained

Published on 02 June 2026

Clinicians’ NHS Dentistry experience (Tiers explained)

A tiered system for individual clinicians will be introduced. Clinicians will be allocated one of two tiers depending on their Scottish NHS Dentistry experience: Tier 1 and Tier 2.

The clinical risk categorisation of individual treatment items and/or combination of treatments will be consistent across both tiers; however, the threshold limits may differ per tier.

Tier 1 will include clinicians who are expected to be familiar with NHS Dentistry in Scotland.

Tier 2 will include clinicians with limited experience of NHS Dentistry in Scotland including those who are:

  • Undertaking VT;
  • Provisionally listed pending completion of mandatory training; and
  • Within 12 months of successful completion of mandatory training or vocational training.

Tiering tables

This table includes a summary of items and their thresholds per tier. If a course of treatment includes items that meet or exceed the threshold for a specific tier, then the course of treatment must be submitted for approval.
